You have the right to access your mental health records. Under both federal HIPAA law and Texas state law, we are required to respond to your request for records within 15 business days. The records will be produced after receipt of any associated fees.
How to Request Your Records
To request copies of your mental health records from Lake Conroe Counseling Center, contact our office and we will send you our Records Request Form through our secure client portal to ensure HIPAA compliance.

Once you receive the form through the portal, complete it with:
- Your full name
- Date of birth
- Contact information (phone number and address)
- Specific records you are requesting (dates of service, type of records)
- Your signature and date
Return the completed form through the secure portal, or you may mail it to:

Fees
We may charge a reasonable, cost-based fee for retrieving and copying your records. You will be informed of any fees before we process your request. We cannot charge you a fee if there is a medical emergency.
Limitations on Access
In rare circumstances, a therapist may determine that access to certain portions of your record could be harmful to your physical or mental health. If this occurs, we will provide you with a written explanation. You have the right to have those records reviewed by another mental health professional of your choosing.
We will delete confidential information about other individuals who have not consented to release their information, but we cannot delete information about you that others have provided.
